A STRUCTURED PROGRAM FOR REAL RESULTS

The Key Journal is a structured program designed to support individuals in building practical skills, developing routine, and increasing independence in everyday life.

Delivered through weekly sessions, the program combines guided journaling, structured discussion and physical activity to help participants build consistency, improve self-awareness, and apply strategies in real-world environments.

HOW THE PROGRAM WORKS

Participants attend structured 2-hour weekly sessions that follow a clear and consistent format. Each session includes guided journaling aligned to weekly focus areas such as goal setting, routine development, communication and social participation. This is supported by facilitated discussion to build self-awareness, emotional regulation and decision-making skills.

Physical activity is integrated into each session to support engagement and routine, while real-world application in community environments helps participants build confidence and independence in everyday settings. The focus is always on applying these skills outside of the session, supporting meaningful progress in day-to-day life.

WHAT THE PROGRAM FOCUSES ON

The program is built around developing key functional skills that support long-term independence. This includes building routine and daily structure, improving communication and social interaction, strengthening confidence in community environments and developing decision-making and self-management skills.

By focusing on consistency and positive habit development, participants are supported to create sustainable change that aligns with their individual NDIS goals.

OUR APPROACH

Each session follows a clear framework designed to support skill development, behaviour change, routine building and real-world application.

This ensures participants are consistently progressing toward greater independence and measurable outcomes over time.

WORKING ALONGSIDE YOUR SUPPORT TEAM

We work collaboratively with support coordinators, psychologists, behaviour support practitioners and allied health teams to ensure a consistent and coordinated approach.

Our program complements existing supports by reinforcing strategies and helping participants apply them in real-life situations between sessions.

PROGRESS & ACCOUNTABILITY

Regular progress updates, including monthly reports, provide a clear overview of each participant’s development. This includes goal progression, skill development, engagement and areas where additional support may be required.

This ensures transparency and alignment across all stakeholders involved in the participant’s care.

WHATS INCLUDED

Each participant receives our psychosocial development journal, which provides a structured framework for goal setting, reflection and habit development. This is a core tool used throughout the program to support self-awareness and track progress.

We provide regular progress updates, including monthly guided video reports, to give a clear overview of each participant’s development, engagement and outcomes. This supports collaboration with support coordinators and allied health professionals.

Participants receive ongoing support between sessions, including check-ins and access to resources, helping reinforce consistency and maintain progress outside of structured sessions.

Physical activity is incorporated into sessions to support routine, engagement and participation. Where appropriate, participants may also access tailored exercise programs and gym-based sessions in a structured environment.
